FBC IGNITE Youth Group Accident Waiver and Release of Liability Form

READ BEFORE SIGNING *

I am the Participants legal guardian and hereby acknowledge and agree that I/we shall indemnify and save harmless First Baptist Church.

RISKS *

I HEREBY ASSUME ALL OF THE RISKS OF ALLOWING THE PARTICIPANT IN ANY/ALL ACTIVITIES ASSOCIATED
WITH FBC IGNITE Youth Group, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

1. Injury Risk *

I acknowledge that this activity may involve a test of a person's physical and mental limits and carries with it the potential for death, serious injury, and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, lack of hydration, and actions of other people including, but not limited to, participants, volunteers, monitors, and/or producers of the activity. These risks are not only inherent to participants, but are also present for volunteers. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.

2. Assuming Risk *

I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said event. In consideration of my application and permitting me to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: First Baptist Church and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers;

(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. I acknowledge that First Baptist Church and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf.

3. Travel *

I understand that occasionally FBC IGNITE Youth Group activities will require the participant to travel on a bus or in an approved volunteer’s vehicle to an event or activity. The participant will only be transported by approved volunteers/staff of First Baptist Church and will never be alone in a vehicle with an FBC volunteer. The greatest care will be taken of the participant but I will not hold FBC or any of its members to blame for any damage.

4. Photos

Over the course of the year, pictures of your child may be posted to different formats of media (ie. Church website, Facebook, Instagram, etc.) I give permission for my child's photo to be posted.
(Please note, should you not agree, that FBC cannot guarantee their picture will not be posted given the public events and potential multitude of cameras, but ministry volunteers and staff will do their best to accommodate your wishes. FBC staff or volunteers cannot be held accountable should your child’s picture appear on social media.)

Agreement and Signature *

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

By applying my electronic signature to this agreement, I agree that my electronic signature is the legally binding equivalent of my handwritten signature on paper. I will not, at any future time, claim that my electronic signature is not legally binding or enforceable. By electronically signing and submitting this agreement, I

1) acknowledge that I have read and fully understand the terms of the agreement;
2) voluntarily agree to be bound by this agreement; and
3) certify that I am 18 years of age or older.
